Baby Dedication Request

Hello HLC Family!

We are absolutely honored to be a part of your family's baby dedication. Here's a few things you should know before making this request.
  • You must be an official member of Have Life Church for your child to be dedicated with us. If you have not officially completed a New Members' Orientation or Life Track classes, please do so before completing this request.
  • Baby dedication is reserved for toddlers aged 2 and younger and will only include the parent(s) of the child.
  • We suggest making this request at least 30 days in advance of your desired date.

Please provide us with a little information so we can schedule your Baby's dedication accordingly.

We will reserve seats for your guests, but we will need a final headcount at least 48 hours prior to the date.
